Case Manager: Brain Injury Community-Based Services

Community Brain Injury Services (CBIS) a private non-profit agency serving individuals with brain injuries in the Metro Richmond, Tri-Cities, South of Richmond, and Hampton Roads’ Peninsula and Middle Peninsula areas, is currently hiring for a Case Manager position to serve our service area South of Metro Richmond.
At Community Brain Injury Services, we serve individuals that have sustained a life altering brain injury. We work with individuals to regain independence, rebuild self-confidence, fully re-engage in their community, and connect to resources needed to live safely and independently.
The Position:
Full-time case managers at CBIS provide case management services to adults with a brain injury. Responsibilities include: conducting intake/assessment of service needs, coordination of services among providers, development/monitoring of service plans, as well as advocacy, consultation, and education for clients, families, and the community. This is a community case manager position that will be based out of a home-based office, though 1-2 days per week in our Richmond Administrative and Case Management Office will be required. This position will be providing services to clients throughout our localities in the South of the Metro Richmond area. This includes: Prince George, Dinwiddie, and Surry Counties, and the Tri-Cities. This position requires the individual to meet clients where they are in the community and throughout the service area. Preference will be given to a candidate that resides in or adjacent to this service area. This position will be provided with an organizational vehicle to utilize for travel throughout the community so maintaining an excellent driving record is a must.
A successful candidate will also be a self-starter skilled at working independently.
An eligible candidate will possess a Bachelor’s degree in social work, psychology, special education, rehabilitation counseling, or closely related field. This position also requires a minimum of two years of case management experience; experience working with persons with disabilities preferred. A valid driver’s license, good DMV record, and access to a reliable personal vehicle are also required. Mileage reimbursement provided when using personal vehicle.
The starting salary for this position is $47,500/annually and includes a robust benefits package.
