The Role:
We are seeking an experienced and forward-thinking Clinical Nurse Specialist (CNS) Primary Care Integrated Pathway to enhance the connection between Community Mental Health Centres (CMHCs), Primary Health Organisations, and local community groups.
In this advanced nursing position, youll play a pivotal role in strengthening the relationship between CMHCs and the primary care sector, aiming to improve access and care coordination for people with mental health needs. This includes developing effective communication pathways, improving clinical integration, and ensuring recovery-focused, whānau-centred care.

As the CNS, you will:
- Build and maintain strong partnerships with CMHCs, GPs, Practice Nurses, NGOs, PHOs, Localities, and community agencies to strengthen integrated care pathways and communication.
- Act as a key contact for non-urgent referrals, manage the CMHC whiteboard, undertake clinical assessments, triage referrals, and coordinate service users into the most appropriate care pathway.
- Conduct comprehensive mental health assessments, lead care planning and referrals, and support wellness planning and health promotion for adults and their whānau.
- Provide expert consultation, education, and support to CMHC clinicians and primary care teams to strengthen collaboration and mental health literacy.
- Lead and contribute to initiatives that improve physical health outcomes, service integration, clinical quality, and continuous service improvement.
- Participate in cross-sector forums, strategic planning, and service development activities to support integrated and responsive models of care.
- Work collaboratively as part of a virtual CNS team under the direction of the Nurse Lead Integrated Care.
Salaries are based on the PSA Mental Health & Public Health Nursing Collective Agreement, Registered Nurse Scale.
About You:
- NZ Registered Nurse with a current Annual Practicing Certificate
- Minimum 5+ years recent clinical experience in mental health (post registration)
- Relevant Postgraduate qualification completed or near completion
- Strong understanding of mental illness, coexisting problems, and evidencebased interventions.
- Full & Clean NZ Drivers Licence
