The Councils County Plan presents a bold vision for how we will reduce the inequity in wellbeing outcomes that residents face across the Borough. Improving health equity is key to driving positive outcomes for residents, keeping people healthier, for longer, in their own homes. An integral element of delivering the Councils Early Intervention and Prevention services is supporting and empowering the community where appropriate and needed to identify, develop and deliver their own responses that contribute to keeping residents Healthy and Well.
The successful candidate will provide highly visible leadership in delivering a community-based response, ensuring that through the Community Capacity Building Officers new groups and networks who want to be part of the Communities Approach are stimulated. This should build resilience, self-sufficiency and sustain independence.
The job purpose is to empower a sustainable community-based provision that delivers early intervention activity at place, helping to keep residents well and therefore reducing the need to access formal council preventative or statutory services.
The job purpose is also to provide a focused and personalised support service connecting vulnerable residents to their communities. This enables residents to build personal resilience and reduce dependency and need for formal services.
