Pay Range: $21.00 – $25.50 per hour*
*Final offers are based on experience, qualifications, skill set, and location.
Sign On Bonus: $2,000 – Full Time (30+ hours), Bonuses paid in two increments; terms apply.

About the Role
The Community Supports Specialist provides community-based and in-home support to adults and transition-age individuals with intellectual and developmental disabilities (IDD) and autism.
Using a person-centered approach, you’ll help individuals build independence, develop life skills, and actively participate in their communities. This role supports individuals living independently (ILS) or with family (IHS) across Weymouth, Quincy, and Milton.
This is a hands-on, field-based role ideal for candidates passionate about human services, social work, and community integration.
What You’ll Do
Direct Support & Skill Building
- Provide one-on-one support aligned with Individual Service Plans (ISPs)
- Teach and support daily living skills (cooking, budgeting, shopping, hygiene, household tasks)
- Promote independence, self-advocacy, and informed choice
- Support participation in community, social, and recreational activities
Community Engagement & Care Coordination
- Accompany individuals to appointments, activities, and community programs
- Assist with transportation and accessing community resources
- Help individuals build relationships and social connections
- Monitor health, safety, and well-being in home and community settings
Case Management & Collaboration
- Support the development and progress of ISP goals
- Attend team meetings and collaborate with families, DDS, and providers
- Coordinate medical, behavioral health, and support services
Documentation & Compliance
- Complete timely and accurate case notes and documentation
- Follow DDS, DPPC, and agency regulations
- Ensure safe, clean, and stable living environments
What We’re Looking For
Education & Experience
- High School Diploma or GED (required); degree in Human Services, Psychology, or Social Work (preferred)
- Experience supporting individuals with developmental disabilities or autism (required)
- Knowledge of person-centered planning and community-based supports
- Familiarity with Positive Behavioral Supports (PBS) and DDS services (preferred)
Skills & Qualities

- Ability to build relationships with individuals, families, and teams
- Flexibility to work varied schedules based on individual needs
Requirements
- Valid driver’s license
- Reliable, insured vehicle and willingness to transport individuals
- Ability to complete required trainings and certifications
