We proudly serve families throughout the Twin Cities with services including companionship, personal care assistance, homemaking, respite care, and post-hospital support.
Position Overview
We are seeking an experienced and relationship-driven Community Liaison to help expand our presence throughout the Twin Cities. This is an ideal opportunity for someone with experience in healthcare, senior services, home care, hospice, social work, senior living, care management, or community outreach.
The ideal candidate enjoys networking, building trusted relationships, and connecting families with valuable resources.
Responsibilities
- Develop relationships with hospital discharge planners, social workers, case managers, senior living communities, elder law attorneys, care managers, and other referral sources
- Conduct in-person outreach visits throughout the Twin Cities
- Represent CivicCare Solutions at networking events, health fairs, and community meetings
- Deliver marketing materials and educate referral partners about our services
- Identify opportunities for community presentations and partnerships
- Maintain a pipeline of referral relationships and potential client opportunities
- Provide a monthly summary of outreach activities and business development efforts
Preferred Qualifications
- Experience in healthcare, home care, hospice, senior living, social services, or community outreach
- Strong communication and relationship-building skills
- Existing professional network within the Twin Cities senior care community preferred
- Self-motivated and comfortable working independently
- Reliable transportation
Compensation
- $1,000 monthly retainer
- $250 bonus for each new client who starts services as a result of your outreach efforts
- Additional performance bonuses available based on client growth
