Responsibilities of the Housing Specialist with the ACT Team
- Supports clients with finding and maintaining safe and affordable housing
- Provides ongoing assistance to help clients apply for, understand and manage their benefits
- Ensure maximum acquisition of entitlements and other forms of assistance for all clients served by the ACT team
- Provide operational administrative support to the ACT team.
- Assists clients in maintaining employment by frequent visits to the work site
Qualifications of the Housing Specialist with the ACT Team
- Bachelor’s degree and one (1) year of case management experience preferred
- Experience working with the population of serious mental illness in the capacity of case management, integrated care coordination, and medical office organization strongly preferred
- Driver’s license, proof of auto insurance, and access to a vehicle for work purposes. Transporting clients in personal vehicle is required.

Eligible for health care (single and family) the 1st of the month following 30 days of employment. Must work a minimum of 20 hours per week to be eligible for benefits and leave accruals. 403(b) retirement plan with employer match available. This is a full-time (40 hours/week) position.
