Medical Lead – Community Aged Care and Aged Care Outreach Service
About the Role
We are seeking an experienced and motivated Geriatrician (or suitably qualified Physician/GP with a special interest in Geriatrics) to join Murrumbidgee Local Health District as the Medical Lead - Community Aged Care and Aged Care Outreach Service (ACOS).
This is a unique leadership opportunity to shape and deliver innovative, patient-centred models of care for older people across a large rural and regional health district. The role combines clinical service delivery, strategic leadership, and service development, with a strong focus on keeping older people well in the community and avoiding unnecessary hospital admissions.
You will play a pivotal role in designing and establishing a contemporary model of care and building a high-performing team that aspires to be innovative, forward-thinking, and recognised as a leader in aged care delivery. This is an opportunity to help create a service that is not only fit for the future but strives to be world class in its approach to community-based geriatric care.
As a key clinical leader, you will work across community, residential aged care, and hospital settings, contributing to the development of progressive models of care including virtual care, hospital-in-the-home, and outreach services.
What You’ll Be Doing
- Provide expert clinical leadership and oversight for MLHD Aged Care and Aged Care Outreach Services.
- Deliver specialist geriatric assessment and consultation in community and outreach settings.
- Lead the development and implementation of innovative, evidence-based models of care focused on prevention, early intervention, and hospital avoidance.
- Work collaboratively with multidisciplinary teams, GPs, community providers, and residential aged care services across the district.
- Contribute to clinical governance, quality improvement, and patient safety initiatives.
- Support teaching, training, and research activities within MLHD.
About You
You are a collaborative and forward-thinking clinician who is passionate about improving outcomes for older people. You bring:
- Fellowship of the RACP (Geriatric Medicine/General Medicine) or RACGP, or equivalent specialist recognition.
- Eligibility for registration with the Medical Board of Australia as a Specialist.
- Demonstrated clinical leadership experience in a complex healthcare environment.
- Experience in service development, quality improvement, and innovation in models of care.
- Strong communication and stakeholder engagement skills, with the ability to work across diverse teams and settings.
- A commitment to education, training, and continuous improvement.

Selection Criteria
- Currently registered or eligible for specialist registration with the Medical Board of Australia. Applicants must have Fellowship of the Royal Australasian College of Physicians (FRACP) or Fellowship of the Royal Australian College of General Practitioners (FRACGP) and/or relevant equivalent specialist recognition.
- Demonstrates effective clinical expertise and leadership in an area of practice relevant to Community Aged Care and ACOS, with ability to adapt clinical practice in accordance with contemporary evidence-based practice.
- Demonstrated clinical leadership and workforce management experience in a complex public health organisation.
- Proven capability to develop and implement complex strategic initiatives including the capability to plan, manage change and improve performance based on service redesign and service implementation methodologies.
- Superior interpersonal skills with ability to build and maintain effective collaborative relationships, to network and engage, collaborate with and influence key stakeholders to achieve business objectives.
- Demonstrated experience in clinical governance quality and compliance processes, and in leading or conducting research, innovation, service redesign or quality improvement initiatives.
- Comprehensive understanding of medical training accreditation processes, and proven experience in the development, growth and delivery of interprofessional education and training.
- Current NSW Drivers Licence, ability to travel as required, and willingness to prioritise the use of virtual modalities wherever possible as an alternative to travel.
