The Adult Social Care Teams support adults to maintain their health and wellbeing, and maximise independence, choice and control.
The Hospital Social Care team is located in Dewsbury District Hospital and supports adults with complex health and social care needs. The team provides assessments, support packages and early reviews to Kirklees residents that have been admitted to hospital to facilitate timely discharge.
The Role
As a Team Manager, you will lead and manage a team of staff including qualified Social Workers. Hospital discharge is a fast-paced environment, and the team works to clearly defined targets and deadlines. Consequently, you will play a key role in ensuring the timely completion of assessments and effective discharge planning. This will also involve you developing positive, supportive relationships with our partner agencies, including the acute hospital trust.
You will also be responsible for monitoring your team’s budget and ensuring that the council is getting best value for care packages and placements.
To support you in your role, there are 2 Deputy Team Managers, who also provide the majority of supervision to the frontline staff.
Change management skills are vital to ensure that changes are embraced positively by your team, whilst fostering a creative ethos to guarantee vulnerable adults are supported with care that is individual to them. You will support your team to adopt enabling approaches and appropriate levels of intervention keeping them to a minimum where required to facilitate individuals independence and positive risk taking as far as possible.
As well as having regular supervision from your Line Manager, you will also have an annual appraisal, as well as a full induction plan when you commence in post.

You will be expected to participate on the managers on call rota. The commitment to this is approximately 1 Saturday every 9 weeks.
You must hold a recognised social work qualification and be registered with Social Work England.

At interview, you will be expected to demonstrate your ability to lead and manage a team, including providing supervision and controlling budgets and resources. You should have extensive experience as a social worker in an adult social care setting, and ideally have experience at deputy or team manager level.
